Who can Apply?
This visa is issued to applicants who would like to visit Singapore for the purpose of medical treatment from a specific doctor/hospital or would like to select a doctor/hospital after reaching Singapore.

Documents Required
Duly completed and signed application Form 14 (See Annexure Singapore-II) by each applicant.

The checklist 4 (See Annexure Singapore-III).

Duly completed and signed Form V-39 A (LOI) (See Annexure Singapore-V) by each applicant including children.

Duly completed and signed Form V-75 (See Annexure Singapore-VI).

In the absence of Form V-75 a letter from the hospital detailing the purpose of visit and the medical condition of the applicant (original and a copy).

2 passport size color photographs. One affixed on Form 14 and other attached with paper clip.

A valid passport of the applicant.

Photocopies of bio-data page, endorsement pages and last 3 pages of the applicant's passport.

Photocopy of the local contact's Singapore Identity Card. Applicant might be asked to provide proof of sufficient funds for stay in Singapore.

Any other document as deemed fit by the authorities.

Note: Applicants with serious illness like cancer can submit their applications by proxy.
Original supporting documents will be returned immediately after checking at the submission counter.
Visa processing time is 3 working days.
