Covering letter from Indian recruiting company detailing applicant's name, rank, vessel's name, arrival date at the port in Netherlands, date of joining vessel, name and address of handling agents in Netherlands.
Appointment Letter or Contract Copy.
Letter / fax of confirmation from the Netherlands agent.
Copies of all filled out pages of all the CDCs.
