Family Reunion Visa (Spouse)
Documents required
1. Completed application form. See Annexure Italy-I
2. Recent passport size photographs (2) with white background not to be stapled with application form.
3. Passport valid for at least 90 days after expiry of the visa (with at least one blank page).  Passport should be issued by the RPO of the jurisdiction from where visa was applied for.
4. No Objection Certificate (Nulla Osta) by competent Police Headquarters in Italy (Original and Copy)
Note: NOC is valid only for six months from issue date; hence the same need to be renewed on expiry. The name in the NOC and in the passport must correspond.
5. Attested photocopy of all pages of the passport (& old if any) of the person who applied for No objection Certificate in Italy.

6. Copy of the Stay Permit (Permesso di soggiorno) duly attested.
7. Ration card copy of the family with its English translation, both duly attested by local authorities and verified by Ministry of External Affairs, India.
8. Air ticket copy
9. The Birth Certificate, Marriage Certificates, Ration Cards and any other relevant documents should be translated in English and verified along with the originals

Notes:
10. Affidavit of the couple (husband and wife) mentioning the existence of marriage, the family composition and specifically mentioning that there is no litigation or divorce and the same is to be attested by residing city magistrate & verified by Ministry of External Affairs, India.

11. The certificate of the spouse residing in Italy may be produced in Italy and got attested by local Italian Authorities in Italy (Comune).
12. Original Marriage Certificate with translation in English, and both attested by the local Authorities & verified by ministry of External Affairs, India.
13. Original Birth Certificate of the spouse with registration number issued by the Registrar of Birth & Deaths along with translation in English to be attested by local Authorities & verified at Ministry of External Affairs, India.
14. If birth is registered at later date as per the Indian custom then get the Birth Certificate made on the order of sub Divisional Magistrate and submit the order copy along with Birth Certificate.
15. In case the applicant’s birth date precedes 01.04.1971 then submit.

(i) Not found certificate
(ii) School leaving Certificate or Middle School Board certificate.
(ii) Affidavit from village head & both parents stating applicants birth date.
16. Birth & Marriage Certificates, School Leaving Certificates must be submitted in original with copy. The photocopies must be attested by local authorities like sub Divisional magistrate / Deputy Commissioner & Home Department and verified by external Affairs Ministry. Photocopy should be made only after attestation of the originals by the local authorities.
17. In case of change in passport details, applicant must submit the proof of residence (bank account, telephone / electricity bill). Applicants are subject to interviews and presentation of other related documents if required.
The originals will be returned once the visa processing is complete.
