Seamen Visa
Documents Required
1. Completed application form. See Annexure Italy-I
2. Recent passport size photographs (2) with white background not to be stapled with application form.
3. Passport valid for at least 90 days after expiry of the visa (with at least one blank page).  Passport should be issued by the RPO of the jurisdiction from where visa was applied for.
4. Indian Shipping Company’s letter.
5. Italian / Sweden Shipping Agent’s fax with details about Flagship of vessel, Exact Time of arrival and departure of the vessel duly attested by Harbor Master Port Captain where the seaman is going to join the vessel.
