6. Confirmed return Air Ticket (Original and copy).

7. Medical Insurance having coverage at least of Euro 30, 000 with inclusion of repatriation flight if required (original and copy)
8. If the applicant is minor then get the application signed by both parents and include approved Leave Application from school.
9. In case of minor applicants, if traveling alone or with only one parent, submit an affidavit duly attested by relevant Home Department where the minor resides.
10. Confirmed hotel booking or Letter from the Italian sponsor
(Lettera di invito). See Annexure Italy-III.

Important: In case of change in passport details, applicant must submit the proof of residence (bank account, telephone / electricity bill). Applicants are subject to interviews and presentation of other related documents of required.
