Familiare Al Seguito Visa (Accompanying Spouse / Parent)
Documents required
1. Completed application form. See Annexure Italy-I.
2. Recent passport size photographs (2) with white background not to be stapled with application form.
3. Passport valid for at least 90 days from visa expiry date (with at least one blank page). Passport should be issued by the RPO of the jurisdiction from where visa was applied for.
4. For this visa, the application for work / study visa and accompanying family visa must be made simultaneously.

5. A “Delega” (delegation) must be signed before the consulate General to nominate a person in Italy to apply for and obtain the No Objection Certificate (Nulla Osta) per visa (per familiare al seguito) on behalf of prime applicant. Person to be nominated for “delega” should be a person in Italy like the sponsor who can apply on behalf of prime applicant to completent Police Headquarters for the NOC for this visa. See Annexure Italy-IV for “Delega”.
6. A declaration (dichiarazione per familiare al seguito), to be made before Consulate General, with copy of identity document, asking for the presence of spouse and minor child / Children in Italy & which attests that they will be economical dependents on the prime applicant. Requisite document can be obtained from Consular office.
7. No Objection Certificate (Nulla Osta) by competent Police Headquarters in Italy (Original and Copy).

Note: NOC is valid only for six months from issue date; hence the same need to be renewed on expiry. The name in the NOC and in the passport must correspond.
8. Copy of all pages of the passport (& old if any) duly attested of the principal applicant who will work / study in Italy.
9. Air Ticket confirmed / reservation printout confirming the booking.
10. Additional document for accompanying spouse in italy:
Marriage Certificate in Original issued by registrar of Marriage, Sub registrar or gram Panchayat, mentioning registration number.

11. Additional documents for accompanying parent in Italy:
Original birth Certificate of child/children issued by registrar of births & Deaths, mentioning registration number.
12. Original certificates affidavits are to be validated by under Secretary of Home department of the State Government translated in Italian and both should be legalized by Consular section of the consulate general.
Notes:
(i)Kindly note that the legalization of civil certificates / affidavits is no longer required from the ministry of external affairs , New Delhi.
(ii) In case of change in passport details, applicant must submit the proof of residence (bank account, telephone / electricity bill). Applicants are subject to interviews and presentation of other related documents if required.
