Instructions for preparing form
1. Please read entire form and explanatory notes before answering.
2. Please print in ink or typewrite.

Universities and colleges in the United States of America require detailed applications for admission which must be accompanied by records and documents that may be expensive or difficult to obtain or to duplicate. To protect yourself from wasting work and money by applying to a college or university which cannot receive you-because it does not offer the course you desire, because its enrollment is already full, because the time for application is past, or because your academic preparation, financial resources or other qualifications are not suitable for the program of study you have indicated. You should fill out this Request for Application Material and send it by airmail to the college or university you have in mind.

The director of admissions of the college or university will carefully review the information in your request and will either send you forms and instructions for filling your application, or he will write you explaining that it does not seem practical for you to apply for the program you have indicated.
